The EHS Ergonomic Program focuses on ergonomic modifications of individual employee computer workstations. Matching funds are available for furniture or accessories recommended as a result of a computer workstation evaluation provided by the EHS ergonomists.
The following are the steps to take to qualify for matching funds:
1. Request an EHS ergonomic evaluation. E-mail email@example.com or call 415.338.2565. A workstation evaluation is required to qualify for matching funds.
2. Purchase the recommended furniture and accessories.*
*Furniture and accessories purchased remain the property of the department making the purchase.
Matching funds are only available for furniture or accessories recommended as a result of a computer workstation evaluation provided by the EHS ergonomists.
EHS will match 50% up to $250** per employee for recommended furniture or accessories.
How to apply?
Complete the SF State Fiscal Affairs Department ‘Transfer Payment Request’ form. Attach a copy of the invoice(s) or receipt(s) for purchased furniture and/or accessories, along with the BI report that corresponds with posted payment and send to Laura Lyons, Admin/Analyst, EHS, firstname.lastname@example.org.
Upon receipt, EHS will transfer 50% of the purchase price up to $250** per employee to the purchasing department.
Funding for this program is limited. Applications will be considered on a first-come, first-served basis. Funding reimbursement will be delayed or denied for paperwork that is incomplete or with insufficient supporting documentation.
