Members of the SF State campus community are encouraged to report hazardous or unsafe conditions so they can be evaluated and corrected before an injury or incident happens. Potentially unsafe conditions include security issues, threats of violence, water collecting on the floor, steam leaks, holes in the floor, etc..
There are several ways to report safety hazards or potentially unsafe conditions at SF State.

- Contact Environment, Health & Safety (EH&S): Office: 415.338.2565 or Email: EH&S
- Use the SFSU app on your cell phone: Select Safety > Report a Hazard
- Submit a paper version: Report a Hazardous or Unsafe Condition form to EH&S in ADM 260 or via email
You may report the issue anonymously if you prefer. However, including your name and contact information will help with assessment of the issue you are reporting and allow us to get back to you with an update. Retaliation for reporting safety issues is prohibited at SF State.
If this is an emergency and requires an immediate response to prevent injury or property damage, call 415.338.2222 from your cell phone, which goes to University Police. A UPD dispatcher is on duty 24 hours a day, seven days a week.
If you observe a fire or smoking equipment or materials, consider this type of condition an emergency.
Online "Report a Hazard" Form
Fill out this online form to report safety hazards, security issues, and potentially unsafe conditions. This helps the university identify hazards so they can be corrected.
When finished, select the SUBMIT button.
The report will automatically be sent to the EH&S office for review and follow-up.
