To keep track of work involving bio-materials requiring a Biohazard Use Authorization (BUA), the University Biosafety Committee has established an annual renewal process. This involves filling out a simple form and attaching relevant information as needed.
This same form is used to document requested changes to the BUA.
DocuSign is used to track the process and signatures. (available in Dec 2023)
