Lab Safety Orientation - New Group Members

Laboratory work poses unique hazards to health and safety that cannot be addressed through online training alone. Information such as location of emergency showers, spill kits, and how to safely shut down specific equipment or experiments is most effective when presented in-person by the lab Responsible Person or designee.

This training should be done before the new lab member starts work. Provide the completed form(s) to EH&S or your department's operations staff within 30 days of the new member's start date.

  1. Download and print the applicable new laboratory member safety orientation form.
  2. Provide an in-person training session reviewing the department and lab-specific information listed on the form.
  3. Document this training by having each trainee sign and date the form in addition to the Responsible Person's signature.
  4. Check off at least the main information categories on the form as you review them. You may check the box next to each line item if you prefer.
  5. Send a copy of this documentation to the EH&S office in Thornton Hall 217, the COSE Dean's Office in TH 320, or via email to lvadura@sfsu.edu.
It is understood that effective safety training is an ongoing process
that continues after the initial safety orientation.
